Team forms: intake and readiness

Two forms collect what you need to know about an athlete:

  • Intake — a one-time profile the athlete fills in when they join: injuries, goals, available days, and anything else you ask.
  • Readiness — a short daily check-in. The app turns it into a readiness score from 0 to 100.

Athletes answer both in the mobile app. You read the answers on the athlete page and on Today.

Organization form and team forms

Every organization has one intake form and one readiness form. Admins edit them under Organization > Settings > Forms.

A team answers the organization's forms until it has its own. On the team page, open Forms > Intake or Forms > Readiness. The page shows the organization form, read-only, with a line that names it.

To give the team its own form, click Customize for the team. This copies the organization form into a team form that you can edit. Answers already given carry over. Later edits to either form don't change the other.

To return a team to the organization's readiness form, choose Back to the organization form from Form actions. Check-ins already answered keep their scores.

Admins can edit any team's forms. A coach can edit the forms of the teams they coach.

Building a form

Both forms use the same builder, with a phone preview beside it.

  • Add a question — on an intake form, pick the answer type: Scale 1–5, Scale 1–10, Number, Yes / No, Pick one, Pick many, Weekdays, Date, Short text, Long text, or Signature.
  • Question actions — move, duplicate, or delete a question.
  • Flag — a readiness answer past a threshold you set flags the day. Flagged athletes lead the list on Today.
  • Gate sessions — a readiness form setting. When it is on, athletes answer today's check-in before a session starts.

When a readiness form already has answers, the builder says so before you change it.

Readiness rules

A readiness rule turns a check-in into a change to today's session. On the team page, open Automation > Readiness rules and click New rule.

  • Who it covers — this team, or every team (admins only). A team rule beats an organization rule.
  • Trigger — the readiness band (Low, or Moderate or lower), a flag, a run of low days, or a score below the athlete's usual.
  • Action — Cut volume, Cut intensity, Skip the session, or Note only. Cuts go up to 50 percent. You can add a note to the athlete.
  • Apply automatically — off, the rule proposes the change on Today and waits for your approval. On, it runs each morning, and you can undo it.
